Report a possible problem

Include the page section, alert title or health-center name, the official source URL, and a short explanation. Do not include private medical information. The public contact channel is listed on the contact page.

Review process

• Compare the displayed record with the current official source.
• Hold high-risk or materially changed content while it is reviewed.
• Correct the normalized record without rewriting the official source.
• Record the reviewer, decision, time, and reason in the private audit log.

Source changes and removals

If an agency changes or withdraws a notice, Health One Hub may hold or withdraw its copy while retaining the internal version history needed for accountability. The original agency remains the authority.
